CNA

Position Summary:

The overall purpose of the certified nurse’s aide (CNA) position is to provide personal care to residents in a manner conducive to their safety and comfort.

Position Responsibilities:

  • Provide personal care to residents in a manner conducive to their safety and comfort consistent with company policy.
  • Promotes and supports the greatest possible degree of independence for residents
  • Advises supervising charge nurse of resident status at beginning and end of shift
  • Contributes knowledge of resident conditions/needs to resident care plans
  • Assists in the implementation of care plans as directed within the parameters of established policy and procedure
  • Observes and reports on resident’s conditions, and documents observations as required
  • Promotes safe/clean environment
  • Complies with requirements of procedures for safe lifting and/or safe transfer of residents per established policies and procedures
  • Must be able to perform moderate to heavy lifting
  • Must be able to walk or stand for extended periods of time
  • Assures that equipment is clean and functional
  • Attends in-service training programs
  • Performs other duties as assigned

Qualifications/Experience Requirements:

  • State certification as a nursing assistant
  • Complete familiarity with duties of a certified nursing assistant
  • Highly developed observation skills are desirable
  • Functional literacy in English is required
  • Tolerance, patience, empathy and concern are desirable

RN Charge Nurse

Position Summary:

The overall purpose of the RN position is to participate in and oversee the provision of resident care as prescribed by the attending physician, consistent with accepted standards of care and within the scope of RN training and licensure. Incumbent also provides direct resident care, and assigns duties to subordinate LPN’s and/or CNA’s as appropriate.

Position Responsibilities:

  • Observes, assesses and reports resident condition/changes, and documentation
  • Assists in collection of data relevant to resident assessment and evaluation
  • Consults with DON regarding changes in resident condition
  • Receives, transcribes, and executes physician’s orders
  • Assures implementation of care plans as directed
  • Oversees/monitors functions and activities of subordinate staff
  • Participates in assigning duties for subordinate staff
  • Contributes to assurance of resident care consistent with accepted standards of care and established policy and procedure
  • Evaluates staff performance and may initiate appropriate action
  • Encourages safety to prevent accidents
  • Other duties as assigned

Qualifications/Experience Requirements:

  • Graduation from an accredited nursing school and a current, valid license or registration (RN) in the state of Oklahoma is required
  • Nursing experience in a long-term care facility is preferred
  • Supervisory experience is desirable
  • Effective communication and interpersonal skills are necessary
  • Functional literacy in English is required

Maintenance Supervisor

Position Summary:

The overall purpose of the maintenance supervisor position is to plan, direct, and control the overall maintenance of the facility physical plant. Incumbents perform or oversee electrical, plumbing, carpentry, heating, ventilation, air conditioning, refrigeration, and other technical tasks.

Qualifications/Experience Requirements:

  • Functional literacy in English is required.
  • Prior experience in institutional maintenance/housekeeping is necessary. The ability to read blueprints/construction drawings is helpful.
  • Excellent physical health is necessary.
  • Prior supervisory experience helpful (larger facilities).
  • Is certified as a Universal Technician as required by EPA 40CRR, Part 82, Subpart F in the use and recovery of refrigerant.

Essential Functions: 

The following represents the essential functions of this job to be used to determine if an individual with a physical of mental impairment can perform these functions with or without reasonable accommodation. The other functions listed may be required of all the other staff in the position.

  • Plans, schedules, and supervises or performs maintenance activities at the facility.
  • Performs various tasks in a timely manner, including electrical, plumbing, carpentry, heating, cooling, and refrigeration repair/renovation/adjustments.
  • Establishes a work-log or checklist for daily, weekly, bi-weekly, or monthly maintenance tasks and/or preventative maintenance actions.
  • Maintains overall appearance of facility physical plant.
  • May obtain drawings, bids for physical plant improvements
  • Assures compliance with applicable governmental regulation including federal, state, local, fire Marshall, and local health department.
  • Establishes and maintains accurate records of maintenance activities. Supervises contract work.
  • Must be able to walk and/or stand for extended periods.
  • Must be able to perform moderate to heavy lifting.
  • Identifies and corrects safety hazards.
  • May maintain lawn, landscaping and building exterior.
  • May clean, strip, seal, wax and buff floors.
  • May clean walls, windows and fixtures.
  • Establishes and maintains record of purchases and inventories of maintenance supplies.
  • Maintains accurate records of carious regulatory inspection (fire inspections), fire drills, etc.
  • Assists in development and management of maintenance budget.
  • Performs other duties/tasks as may be assigned,

Charge Nurse- LVN

Position Summary:

The overall purpose of the Charge Nurse – LPN/LVN position is to participate in and oversee the provision of resident care as prescribed by the attending physician, consistent with accepted standards of care and within the scope of LPN/LVN training and licensure. Incumbent also provides direct resident care, and assigns duties to subordinate LPN= S/LVN=s and /or nursing aides as appropriate.

Qualifications/Experience Requirements:

  • Graduation from an accredited nursing school and a current, called license or registration (LPN/LVN) in the state where employed are required.
  • Nursing experience in a long-tern care facility is preferred.
  • Supervisory experience is desirable.
  • Functional literacy in English is required.
